required for Counseling:
- Original Admit Card of UPCMET - 2011.
- Marks Sheet/ Rank Card of UPCMET - 2011, downloaded from the UPUMCWA website.
- Original Marks Sheets/ Certificates of 10th, 12th along with one duly attested
photocopy set of the same.
- Counseling Fee of Rs. 750/- through Cash/ Demand Draft in favour of "UPUMCWA" payable
at "Bareilly".
- One Passport Size photograph.
- Caste Certificate in case of OBC/ SC/ ST candidates (in case of OBC the same must
be issued after April 01, 2011).
Note: Candidate may report for counseling with
the requisite documents at any of the aforesaid centres.
